Building A Healthy Community

Suburban Hospital’s footprint has long been rooted in community health. In living out its mission of improving health with skill and compassion, the services of Suburban’s workforce expand beyond the walls of the hospital and deep into Montgomery County, nurturing a healthy, vibrant and resilient community.

Suburban Hospital’s framework for building a healthy community is deliberate and purposeful. Aligning health priorities -identified through the triannual Community Health Needs Assessment (CHNA) process- with the hospital’s area of expertise and fostering cross-sector collaboration, the hospital seeks to achieve significant and lasting changes.

Developed every three years, in response to the hospital’s latest community health needs assessment and adopted by the hospital Board of Trustees, Suburban Hospital’s implementation plan is a roadmap for addressing community health priorities and improving health outcomes. The plan considers where resources will generate the greatest impact by applying three critical lenses:

Healthy Equity

Reduce gaps in access to preventive services for vulnerable populations.

Access to Care

Increase access to specialty care for low-income and uninsured residents

Healthy Behaviors

Promote healthy and active living.
