Make a LASIK Evaluation Appointment

Welcome to The Wilmer Laser Vision Center's on-line appointment request system. To make a LASIK evaluation appointment, please enter the required information in the spaces below, then click on the SUBMIT button at the bottom of the page. We will contact you, usually within five business days, to propose an appointment date and time.

NOTE: Soft contact lenses must be removed two weeks prior to your evaluation; rigid gas-permeable and hard contact lenses must be removed at least three weeks prior to your evaluation. The comprehensive evaluation usually takes around 1 - 2 hours and your eyes WILL BE dilated during the evaluation. The cost for the evaluation is $95.00 and is not covered by insurance.
