IRB and Office of Human Subjects Research (OHSR) Holiday Schedule IRB meetings will be canceled the week of December 24th - 28th. The OHSR will be open a half-day December 24th and closed on December 25th. We will also be open a half-day on December 31st and closed on January 1st.
A new version of the Combined Informed Consent/Authorization
Template was posted in October. All new and revised consent forms must
use this template after January 1, 2008. Consent forms received after
January 1 on an outdated template will be returned and the application
will not be processed until the correct consent forms are submitted. |
