Flu Vaccine Shortage

Johns Hopkins US Family Health Plan members who are unable to obtain seasonal flu shots at their primary care office due to the recent shortage of the vaccine may now obtain the vaccine from any provider. If immediate payment is required, please submit a receipt, bill or other proof of vaccination and payment along with a completed US Family Health Plan reimbursement form to the address shown on the top of the form. The Claims Department will process the reimbursement request and refund up to $25 to the member.

Since the H1N1 vaccine also is anticipated to be in short supply, US Family Health members can follow the same process for obtaining the H1N1 vaccine. Please keep in mind that providers are following CDC protocols and are assigning priorities to patients prior to vaccinating. Because this vaccine is paid for by the state of Maryland, members should only be charged an administration fee and not a fee for the vaccine. The Plan will reimburse members up to $15 for the administration of the H1N1 vaccine upon submission of a receipt, bill, or other proof of vaccination and payment to the address shown at the top of the claim form.
