Bringing Your Medical Team Closer

Johns Hopkins is always looking for ways to lessen the geographical distance between the members of your medical team – your team located at Hopkins and at your neighborhood doctor’s office.  The Comprehensive Transplant Center’s “automated patient notes” system, created in 2003, helps do that.  Through this system, Hopkins physicians now send medical summaries of your outpatient clinic visits and inpatient stays at Johns Hopkins to your local doctor.  This can include the most recent summaries of test results and descriptions of Hopkins clinical recommendations.

Amazingly, medical summaries of your Hopkins consultations can arrive at your physician’s office before you do.  When your physician has these summaries, she or he is able to make recommendations about your healthcare with the most complete information available at the time.

Not only does this save you from having to arrange for tests to be sent or to have duplicate testing, but it can allow you and your doctor to address your medical concerns earlier.

“Automated patient notes ensures that the information from your most recent visit to JHH will reach your primary care provider in a timely manner.”
