Criminal Background Check

Criminal background checks are required for all adults/mentors who  are 18 years or older and involved with a JHUSOM program/group that interacts with youth/children.  There may be a charge for this service.   Please complete all of these forms and email them to SOMYouthPrograms@jhmi.edu or return them to the Office for Student Diversity, Miller Research Building, Suite 137, 733 N. Broadway, Baltimore, MD 21205.

1. Informative: Johns Hopkins Child Safety Policy
2. Informative: Your Rights Under the Fair Credit Reporting Act
3. Form: Disclosure and Authorization (to be completed by summer program participant and returned to the director or administrator of the summer program; please keep a copy for your records.)
4. Form: Criminal Background Report Form (to be completed by the director or administrator of the summer program. This report form will be returned to whoever completes this form that indicates the results of the criminal background check.)
