Biennial Meeting and Reunion Weekend Registration

Registration for the 2013 Biennial Meeting is now open.  Please click here to access the PDF registration form and information brochure.  Completed forms with payment should be forwarded to the Office of Continuing Medical Education (OCME).  Mailing address, fax, and phone number are listed at the bottom of the registration form.

Registration deadline is May 30, 2013.

Invitations, which include registration and payment information, for the All Classes Reunion Dinner will be arriving by mail.  Please complete the RVSP registration card by May 27 and return to us.
