Initial enrollment application in the JHUSOM 403(b) retirement plan for eligible Postdoctoral Fellows can be made by submitting the following forms to the Registrar’s Office, 147 Broadway Research Building, 733 N. Broadway, Baltimore, MD 21205:
- a signed original Authorization for Reduction of Taxable Compensation and Investment Option Allocation Agreement for Income Deferral 403(b) Retirement Plan for Postdoctoral Fellows form; AND
- relevant investment company enrollment applications
Enrollment packets with all applicable forms and eligibility guidelines are available in the Registrar’s Office. The effective date of the agreement is the first day of the month following receipt of all applicable forms. Changes in salary reduction for an existing JHUSOM 403(b) retirement plan can be made by submitting a signed original Authorization for Reduction of Taxable Compensation and Investment Option Allocation Agreement for Income Deferral 403(b) Retirement Plan for Postdoctoral Fellows form to the Registrar’s Office. Forms are available in the Registrar’s Office and online at http://hrnt.jhu.edu/benefits/forms.cfm#retirement. The effective date of the change is the first day of the month following receipt of the form.
