New Patients

If you are a new patient:

  1. One of our new patient coordinators will return your call and take all the necessary information.
  2. You will be instructed to bring:
    • Your insurance card
    • ID card
    • Other information
  3. Before the appointment, all pertinent medical records from referring physicians should be faxed to (410) 997-6019.

How do I request my medical records?

A medical records request can be made by calling the office or contacting us through the mail or by email. The request should include first name, middle name, date of birth and phone number.

As a service to our patients, there is no charge for records sent directly to another physician’s office. If you would like your own copy, there is a charge for each copy of your medical record requested. This charge is set by the Maryland State Medical Association (MedChi). We will inform you of the charge when chart is ready for pick up. Please allow us a minimum of seven day’s notice to prepare your copy. We will call when they are ready to be picked up. Due to HIPAA requirements, you will be expected to sign a written request for your medical records.

Prescription refills

We will be pleased to call to renew or refill prescriptions at your pharmacy. We ask that you tell us the name and dosage of your medication and the name and phone number (and the fax number, if possible) of your pharmacy. Prescription requests by email, telephone or fax will be sent to the pharmacy within 24 hours.
