Have you observed a staff member who gave extra special effort to deliver the highest quality Service possible? We want to recognize that person's efforts. Just fill out the form to tell them. Once you have completed the form, you can drop it in any Shining Star box located throughout the Hospital or Outpatient Center.

If you already know the individual's supervisor, you can give the form directly to the supervisor to present to the employee.

Once an employee has earned 5 Catch a Shining Star awards, he or she is eligible to receive a gift from the Service Excellence Department. The Verification Form verifies that the manager has reviewed all 5 awards and that the employee is eligible for the gift.
