CHANGE IN NAME, EMPLOYER, DEPARTMENT OR TITLE
Any employee needing a new badge to reflect a change in Name, Employer, Department or Title must present a completed JHMI BADGE REQUEST FORM from their office of record along with the badge being replaced. There is no charge for this badge reissuance.

Note: The name used on the ID badge will be the Legal Name of the individual requiring the badge - NO NICKNAMES WILL BE USED.
