Our Process: Coordination With Your Treating Physician
Written permission must be obtained from your local physician to request a medical second opinion from Johns Hopkins Otolaryngology-Head and Neck Surgery. After completing required forms, you will mail us copies of pertinent medical records and diagnostic test results. A fee will be charged to a credit card prior to the records being reviewed. A formal letter containing the Johns Hopkins physician's assessment and recommendation(s) will be mailed or faxed to your local physician.
Non-Participating States
In accordance with state statutes, this service is not available for patients residing in:
- Iowa
- Louisiana
- Maine
- Michigan
- North Dakota
