The physicians and specialty clinics
identified in this directory will assist you and your patients.
The Main Admitting Office will accept admission requests, emergency
or non-emergency, from physicians who have admitting priviledges
at the Hospital. You may be directed to one of the satellite
offices for admissions to the Childrens Center, the Oncology
Center or the Department of Psychiatry and Behavioral Medicine.
The Hopkins Access Line also may be used to make such arrangements,
or for referrals to a Hopkins physician.
The following information should be assembled in advance to
help us expedite the best possible medical care for your patient:
- Date of admission
- Patient's full name, middle initial
- Patient's current address, phone number
- Patient's date of birth, mother's maiden name
- Patient's sex, race
- Patient's Hopkins medical record number, if any
- Previous Hopkins admission date
- Physician's name/service specialty/code number
- Provisional diagnosis (not procedure)
- Nature of admission (elective, urgent, emergency)
- Required preadmission tests
- Estimated length of stay
- Accommodation desired (private or semi-private room)
- O.R. date/time/procedure
- Plan of treatment, including medical justification for
preoperative day
- Source of admission (patient is to come from home, hospital,
nursing home)
- Referring physician name/address/telephone number
- Insurance information, primary and secondary

requirements or estimate of hospital costs, call 410-955-6055
or 410-955-6056, Monday through Friday, 8 a.m. to 5 p.m.
Reservations for elective admissions are contingent on the
availability of beds on the day of admission. Requests for
single rooms are honored, whenever possible. Every attempt
will be made to contact the patient by either mail or telephone
to collect all additional pertinent information and provide
information to the patient regarding admission.

Special Admitting Information
Children's Center
Two routes are available for admitting patients to the Children's
Center. Choose the route that is most convenient.
- Patients with acute problems may be evaluated by the pediatric
emergency staff in the Pediatric Emergency Room, followed by telephone
consultation with the referring physician to determine management.

- Physicians with admitting privileges can admit their patients
directly by calling the Children's Center Admitting Office at
410-955-8756, Monday through Friday, 8 a.m. to 5 p.m. Any other
day or time, call 410-955-6190.
Department of Psychiatry and Behavioral Medicine
- This service accepts voluntary admissions for patients 14 years
of age and older with primary DSM III diagnoses.
- Patients must be referred by a physician or mental health professional
and admitted through the Psychiatry Admitting Office.
- Patients who may require emergency admission are evaluated in
Johns Hopkins Hospital's Psychiatric Emergency Room.
