ADMITTING

The physicians and specialty clinics identified in this directory want to assist you and your patients. The Admitting Office accommodates admission requests, emergency or non-emergency, from physicians who have admitting privileges to Hopkins Bayview.

Admission to the inpatient psychiatric unit and the Center for Chemical Dependency is by direct referral to the unit.

The Admitting Office is open 7 a.m. to 11 p.m., seven days a week. Reservations can be made Monday through Friday, 8:30 a.m. to 5 p.m. For admissions after 11 p.m., contact the Emergency Department at 410-550-0350.

For information related to insurance, preadmission authorization requirements and estimate of hospital costs, call 410-550-0833 or 410-550-0834 Monday through Friday, 8 a.m. to 4:30 p.m.

The following information is needed. Forms are available by calling 410-550-0836.

• Date of admission
• Patient's full name, middle initial
• Patient's current address, phone number
• Patient's date of birth
• Patient's sex, race
• Patient's Hopkins Bayview history number, if any
• Previous Hopkins Bayview admission date
• Physician's name/service, specialty/code number, admitting physician’s name
• Provisional diagnosis (not procedure)
• Nature of admission (elective, urgent, emergency)
• Required preadmission tests
• Estimated length of stay
• Accommodation desired (private or semi-private room)
• Plan of treatment, including medical justification for preoperative day
• Source of admission (patient is to come to Hopkins Bayview from home, hospital, nursing home)
• Referring physician name, address
• Insurance information, primary and secondary.
