Scheduling an Appointment

An appointment can be requested by calling 443-287-3313.

If a provider would like their patient to have overnight sleep testing prior to their clinic evaluation, a referral form must be completed and faxed to our office for review. Office notes including a history and physical and any necessary insurance referrals must also be included.

New patients should bring a completed Sleep Questionnaire to their clinic or laboratory visit.
