- What you have been told about your illness, so we can best direct your appointment.
- Medical insurance information
If you have had diagnostic testing (pulmonary function studies, chest x-rays, CT scans, biopsies, etc.) performed elsewhere, please bring the results or x-rays with you. This will avoid unnecessary delay or repeat testing.
