Child Case Management

Johns Hopkins Bayview Medical Center

The Child Case Management Program helps children with psychiatric illnesses and their families access and fully unitize resources available to them. Case managers help prioritize needs, serve as advocates for patients and families as well as service planners and schedulers. Professional staff help coordinate services from multiple providers and programs. They conduct in-depth home assessments and develop individual case-management plans that identify problem areas, strengths, and treatment goals. Patients and their families must have Medical Assistance or be eligible for it.
