Lauren Graham's Bio

Lauren Graham

 Lauren is a third year medical student taking a year off to pursue a MPH in the School of Public Health and is interested in research related to aging, health service delivery, chronic care models and health care disparities. She works with Dr. Alicia Arbaje on care transition research using the Geri-FITT intervention piloted at Johns Hopkins Hospital working to improve transitions from hospital to home and decrease readmissions rates in elderly adults. Her goal is to identify factors that increase the likelihood for difficult transitions of care as well as determine strategies that can improve care in the geriatric population.
