Before You Arrive
At the time of scheduling, you should be asked for all demographic and insurance information. Plan to arrive 15 minutes before your scheduled appointment if you have preregistered and 30 minutes if you have not.
What to Bring
On the day of your appointment, you will need the following:
- All prior medical records, including any x-ray or CAT, MRI scans and any available glass microscope slides
- Insurance cards and forms
- HMO/PPO authorization forms
- Social Security number of the insured person, as well as the name and address of that person's employer.
*Also, you may need to bring your medical records, X-ray films, or prior test results if you are being referred to Hopkins by another physician.
Outpatient Center staff will verify whether you have pre-registered, and, if not, will register you at this time.
Financial and Billing Services
The department's billing staff will handle all the paperwork required by your insurance company for its portion of your bill.
Payment is expected at the time of service for your portion of the charges. The scheduling staff should be able to estimate the amount due.
Balances from services in other departments or The Johns Hopkins Hospital are also expected in advance of receiving service from our Department. In addition, payment is expected up front for services, such as hearing aids, which are excluded from insurance coverage. Payment plans are available based on financial need.
