Request an Appointment

To communicate about an appointment, please call Carol James at 410.614.4449, fax her at 410.955.0626, or email her at cjames@jhmi.edu providing the following information:

  • Patient demographics – full name, date of birth, address, phone number(s), e-mail address, fax numbers, and type of medical insurance.
  • A full description of the facial pain to include exactly what the pain feels like, how long it lasts, what brings it on or makes it worse.  What medication has had the best effect on the pain (even if it didn’t take it all away, and even if it gave you a lot of undesirable side-effects)?  Have you ever had a surgical procedure for this condition, and if so, what was it and how well did it work?
  • Have you had an MRI of the brain?  What did it show and do you have it to bring with you for a consultation?
  • What other medical problems do you have? 
  • Please provide a complete list of all medications currently being taken for any condition (both prescription and over the counter medications, as well as vitamins or herbal supplements).
  • If possible, please have your neurologist or internist (whoever has the best knowledge of your TN) fax your pertinent records to our office.

Having this information will assist us in knowing what kind of procedures you may qualify for, which trigeminal physician is best able to assist  you, and if our hospital or our surgeons participate with your insurance plan.  It may be necessary for us to see a copy of your insurance card (front and back) so that we may assist you with insurance issues.
