Prepare for an Appointment

To help facilitate your appointment, we ask that you bring the following items with you:

• Prior to your appointment, you will receive a new patient questionnaire in the mail. Please fill this out in advance.

• Your MRI scans (with contrast) 
• If you have ever had an operation to treat your trigeminal neuralgia, please bring a copy of the operative notes from your previous surgeon.
