Patient Referrals

We request that physicians wishing to refer a patient to the Johns Hopkins Peripheral Nerve Surgery Center compile complete medical records on the patient including:

• Medical history
• Diagnosis
• Any tests or procedures that have been completed related to their diagnosis.
