Refer a Patient

To refer a patient to see a member of the pediatric neurosurgery team, we request the following information:

• Demographics of the patient, such as name, address and phone number
• Insurance information
• Details about the nature of the problem
• Pertinent records, such as head circumference, clinic notes and scan reports
