New patients must be referred by a physician (unless you have already received a neuromuscular diagnosis).
Our staff will need the following information prior to scheduling an appointment:
- Contact information (name, address, and phone numbers)
- A copy of the front and back of your medical insurance card(s)
- Medical records pertaining to your neuromuscular diagnosis or symptoms. This may include physicians’ office visit notes, a physician’s letter of referral, lab results, and/or reports from EMG, MRI, CT or other tests. Please do not send radiology films – bring them with you to your appointment.
Once we have received your medical records, they will be reviewed by one of our physicians. Our staff will contact you after your records have been reviewed.
