Prepare for an Appointment

Prior to your scheduled appointment, you will receive a letter confirming the date and time of your appointment. We ask that you bring:

  • A list of your current medications, including dosage and reason you are taking each medication
  • Any medical records you have not already sent to us, relevant to your symptoms and diagnosis
  • If you have radiology films, please bring them with you to the appointment. You do not need to send these to us before hand.

Insurance Verification and Referrals
We recommend that you contact your insurance company prior to your appointment to verify benefits for the office visit, lab work, radiology, and other potential services. If your insurance company requires a referral, it should be faxed to your physician's office prior to your appointment in the Memory Disorders Center.

 

Carefinder
Hopkins clinicians recommend the Alzheimer's Association web resource known as Carefinder, It outlines how to plan ahead for patients with memory problems, and identifies care options, support services, and guidelines for how to coordinate care for persons with memory disorders. This interactive web-based tool permits you to identify resources that fit your needs.
