Prepare for an Appointment

Please bring the following to your scheduled appointment at the Johns Hopkins Headache Center:

  • Films or CD-ROMs of any recent MRIs and CT scans of both the brain and the neck, along with reports
  • Copies of any initial office consult notes about your condition
  • Any relevant recent test results including lumbar puncture (spinal tap) results
  • A list of medications you use, including doses, start date, reason for taking the medication

To better serve you, please complete our new patient questionnaire that will be mailed to you two weeks prior to your appointment.  Please bring the completed form with you on the day of your appointment. The questionnaire will provide us with a detailed medical history so we will be able to spend more time discussing treatment options with you on the day of your evaluation. If you do not receive the questionnaire, please contact Beverly Wesko at 410.550.ACHE (2243).

We are unable to see any patient without a referral and insurance authorization completed in advance of the visit. Be advised that referral authorizations sometimes expire and require renewal. You may wish to check with your referring provider to be sure the proper paperwork has been sent. You will be required to submit all co-pays at the time of the visit.
