Thank you for considering the Charcot-Marie-Tooth Center at Johns Hopkins. Our team will need to see the following information prior to scheduling an appointment for you:

• Contact information (name, address, and phone numbers)
• A copy of the front and back of your medical insurance card(s)
• Medical records pertaining to your diagnosis or symptoms. This may include physician office visit notes, a physician letter of referral, lab results, and/or reports from other tests.

Once we have received your medical records, they will be reviewed by one of our physicians. We will contact you after your records have been reviewed.
