Non-Member Service Request Form	Use services of Microscope Facility; supervised use only.	Principal Investigator or Accounts Administrator must sign; return to Microscope Facility
Service Request Checklist	Enumerate requirements of service request	Complete prior to advisory meeting with Facility staff
Lab Group Membership Form	Join the Microscope Facility (by lab groups); contract agreeing to 12-months of membership fees; specifies contact information and personnel	Principal Investigator must sign 12-month commitment; return to Microscope Facility
User Enrollment Form	Individuals within each lab group who want to use the Facility	Return to Microscope Facility
SOM Card Access Form	Request 24/7 access to equipment rooms; certified member lab users only	Return to Microscope Facility; allow 2 weeks for activation
