To provide the highest quality medicine for our patients, Johns Hopkins Bayview Medical Center offers care teams, comprised of physicians, nurse practitioners, physician assistants and case managers, that are available for patients 24 hours a day, 365 days a year. Each team is dedicated to the well-being of their patients, and serves as an example of the clinical and academic excellence for which Johns Hopkins Medicine is recognized.
Each day, patients are seen by at least one team member. Although the team members may change from one day to the next, they are in constant communication with each other about medical conditions and needs, as daily team meetings and a secure online database help us deliver uninterrupted high-quality care.
We also highly value communication with primary care providers. We try to include your primary care provider or another physician from his or her practice whenever possible, and always send him or her a detailed discharge summary at the end of your hospitalization.
After you are discharged, most questions should be directed to your primary care provider. However, if you need to contact a care team member, please call 410-550-0100 and ask to be directed to the unit on which you were hospitalized.
