Paper-Based Tools

The Center for Innovation has made these paper order sets free for your use and modification. Contact us to obtain copies.

It is important to note that these paper-based tools are copyrighted to Johns Hopkins Medicine, and also that they are several years old at this point. Guidelines and best practices may have changed, and we advise that these be used as a guide for developing and testing your own tool, rather than using these documents in patient care as they currently stand. At Hopkins, we have begun using our electronic VTE risk assessment tools, based on algorithms developed for our provider order-entry system.

Electronic Risk Assessment Tools

We are currently exploring ways to bring our electronic risk-assessment tools to other health systems that can implement them. If you are interested in learning more about these tools, please contact us with your information and we will follow up with you personally.

Data Reporting and Safety Tracking Services
