It is important to note that these paper-based tools are copyrighted to Johns Hopkins Medicine, and also that they are several years old at this point. Guidelines and best practices may have changed, and we advise that these be used as a guide for developing and testing your own tool, rather than using these documents in patient care as they currently stand. At Hopkins, we have begun using our electronic VTE risk assessment tools, based on algorithms developed for our provider order-entry system.
