What to Bring to Your Appointment

Patients should bring the following items to their appointment:

Driver’s license or state ID card
Insurance cards
Physician’s order (prescription or requisition)
Primary care doctor or specialist’s referral form
Complete name, address, phone and fax number of referring physicians and other doctors to receive imaging results
List of current medications and known allergies
CDs obtained at other sites
Time of Service Payments (co-pay, co-insurance, self-pay)
