If you are an employee requesting a religious accommodation, please complete the appropriate form below (there are separate forms for JHU and JHH employees) and return it to your supervisor.  If you are requesting a medical exception, please complete the appropriate form below (there are separate forms for JHU and JHH employees) and submit it to the Occupational Health Services office.
