Gilchrist Hospice Care, the largest hospice organization in the State of Maryland, is a provider of choice in the Baltimore metropolitan area for patients of the Johns Hopkins Health System through its Johns Hopkins Home Care Group.
Since accepting its first home care patient in 1994, Gilchrist Hospice Care has provided quality, compassionate end of life care for more than 20,000 patients and their families in their own homes, in long term care facilities and at its acute inpatient center, Gilchrist Center.
Patients admitted to Gilchrist Hospice Care receive a wide-range of physical, social and spiritual support designed to ease the burdens that come with a terminal diagnosis. Once under the care of Hospice, Hopkins patients can choose to maintain their relationships with their current medical doctors while also receiving comprehensive care from a team of hospice workers that includes registered nurses, social workers, hospice aides, chaplains and volunteers.
Gilchrist Hospice Care serves patients in Baltimore City and Baltimore, Carroll, Harford and Howard counties. The nonprofit hospice organization is certified by Medicare, licensed by the State of Maryland and accredited by the Joint Commission on Accreditation of Healthcare Organizations.
Coordinators from the Johns Hopkins Home Care Group are on-site at Johns Hopkins hospitals to facilitate hospice referrals and consultation visits with liaisons from Gilchrist Hospice Care. These consultations are recommended and encouraged so that patients and their families can consider all of their options for end of life care early in the planning process and make the decision that best suits their needs.
