The Washington Home (TWH), a nonprofit, Washington, D.C.-based health care facility, provides community-based hospice and palliative care to patients of The Johns Hopkins Health System through an affiliation with the Johns Hopkins Home Care Group (JHHCG).
TWH's Community Hospices, including Maryland Community Hospice, Virginia Community Hospice and Hospice of Washington, are a preferred hospice provider for patients of the Johns Hopkins Health System in Maryland, Washington D.C., and Northern Virginia.
Unlike many other hospice providers, Community Hospices will accept terminally ill patients from The Johns Hopkins Health System, regardless of their ability to pay, under a mutually developed system of clinical and operational protocols designed to enhance timely access to needed services and improve care at the end of life. Hopkins' patients will continue to have a choice of providers and treatment options.
Johns Hopkins Home Care Group coordinators are an on-site resource in Johns Hopkins hospitals to facilitate hospice referrals and Community Hospice liaison consultation visits. Hospice consultation visits are encouraged to discuss hospice options with patients and care givers early in the “end of life” planning process.
