To receive your health records, download the following form:

  1. Authorization for Release of Health Information
  2. Print the form and fill it out completely. Make sure to sign and date the form. We cannot release your records without that information. We may return incomplete forms to you to finish.
  3. You may request records for someone else, if you are the patient’s legally appointed representative, such as court-appointed guardian. See the form for details about attaching proof of your authority to act on the patient’s behalf.
  4. Return the completed form with any necessary attachments to the address below. We will send your records within 10 to 14 business days.
  5. Mail your request to:
