Summary of Trial:To determine if an algorithm can be developed based on pre-implant clinical variables to predict responses to CRT therapy. Any patient who receives an FDA approved SJM CRT-D is eligible for enrollment.

Inclusions:

New York Heart Association Class III or IV

QRS > 130 msec

Ejection Fraction < 35% within last 6 months

Clinical indication for CRT-D therapy

Exclusions:   

Unstable angina

Patients with an ICD or pacemaker

Patients that require revascularization
