The Johns Hopkins Fellowship in Hospital Medicine is one of the nation's premier hospitalist fellowship training programs for academic hospitalists and future leaders of hospitalist programs. Fellows are trained and mentored by nationally recognized faculty of Johns Hopkins University School of Medicine. The curriculum is based on the Society of Hospital Medicine's core competencies.
Goals and Mission
The mission: To create productive members and directors of academic and community hospitalists programs, depending on the individual fellow's goals. The goals of the curriculum are to advance fellows' knowledge, attitudes and skills in the domains of clinical medicine, health care systems, professionalism, education and research. The curriculum will prepare the fellows to become leaders in clinical and educational program development, agents of systems change, role models of professionalism and effective researchers.
Objectives
Our curriculum is based on the Core Competencies in Hospital Medicine, defined by the Society of Hospital Medicine (SHM) in 2006. Knowing that all physicians graduating from an internal medicine program are thoroughly trained in clinical medicine and procedures, but recognizing that residents receive very little education in health care systems, our curriculum content emphasizes the following topics: consult medicine, care of elderly patients, palliative care, evidence-based medicine, practice-based learning and improvement, team approach and multidisciplinary care, patient safety and quality improvement, leadership, patient handoff and transitions of care, financial management processes, information management, medical ethics and professionalism, equitable allocation of resources, risk management, communication, education and research.
The five overall goals of our curriculum incorporate the 24 Core Competencies:
Clinical Medicine: Fellows will advance their proficiency in providing clinical care for hospitalized patients, with emphasis on the following areas: evidence-based practice, palliative care, care of elderly patients, medical consultation, comanagement with surgical services and practice-based learning and improvment. Additional training in critical care medicine is available.
Health Care Systems: Fellows will develop the knowledge, attitudes and skills necessary to help health care systems change in ways that improve the processes and outcomes of patient care, with emphasis on the following areas: patient safety, quality improvement methods, transitions of care and risk management.
Professionalism: Fellows will develop the knowledge, attitudes, skills and behaviors that constitute professionalism, including: altruism, respect, cross-cultural sensitivity, accountability, confidentiality, communication and shared decision-making, integrity, compassion/empathy, duty, recognizing and managing conflicts of interest, self-awareness and commitment to excellence and ongoing professional development.
Education: Fellows will acquire teaching and curriculum development skills that incorporate adult learning principles, which will allow them to become leaders in hospital medicine education.
Research: Fellows will develop skills in research design, instrument development, data collection and analysis, research collaboration and the dissemination of findings. The acquisition of these skills will promote success, satisfaction and advancement in academic medicine careers.
Program Details
Duration of Fellowship: Two-year program.
Sites: The Johns Hopkins Bayview Medical Center; Johns Hopkins Hospital; and Greater Baltimore Medical Center.
Number of fellows: 1-2 per year.
Eligibility: Successful completion of an ACGME-approved Internal Medicine residency.
Visa status: J-1 and H-1 visas are offered.
Clinical time: 30% per year.
Educational Methods: Clinical rotations; required readings; quality-improvement projects; participation in academic conferences; and participation in courses such as faculty development, curriculum development and some masters in public health classes. The fellow will complete many of the objectives independently, with skilled and dedicated mentors available for questions, advice and guidance. Mentors will encourage fellows to reflect regularly, in order to stimulate their professional and personal growth and development.
Clinical Rotation Opportunities: Include the palliative care service and hip fracture service at Bayview; the Hospitalist Unit at Hopkins Hospital; the CIMS service at Bayview; the Hopkins comprehensive consult service; and critical care rotations for those who desire additional training.
Hospital Committees: Fellows will join our faculty to serve on committees within the hospital.
Research Meetings: The fellows will present their ideas and work, such as new practice guidelines or QI initiatives, at faculty meetings and research in progress conferences attended by GIM research faculty and Geriatric's fellows and faculty. Fellows will be encouraged to participate in national professional conferences.
Mentorship: Mentors will be assigned to the fellow for each of the core competencies and/or rotations (e.g., a mentor for QI work, mentors for billing/financial management, or a specific clinical mentor for each clinical rotation). Most of the mentors will be faculty members in Hospitalist Medicine, the Division of General Internal Medicine, and Geriatrics. The mentor will work closely with the fellows and will provide ongoing feedback. Quarterly meetings between the fellow and the fellowship committee (which will include the program directors and faculty within the divisions of hospitalist medicine) will address the fellow's progress through the fellowship program.
Teaching Opportunities: Fellows will be encouraged to enroll in the teaching skills and curriculum development course offered at Bayview. Fellows will teach fourth-year medical students (sub-interns) and visiting residents on the Hospitalist Unit at Hopkins Hospital and interns on the consult service.
Leadership: Fellows will attend the SHM leadership academy level I and II. They will learn skills such as leading and managing change, understanding business drivers for hospital survival and success, effective communication, and conflict and negotiation for hospitalists.
