Clinical Nurse Practitioner

 Allegra Hamman, CRNP

Patients are seen by appointment only. Please call 410-583-2774 to schedule an appointment. Please call at least 24 hours in advance if you need to cancel your appointment, so we can offer that time to another patient.

Patient Call-In Times

Patient call-in times are protected time in your physician’s schedule. This is the most effective way to speak directly with your personal physician to review test results and/or ask non-urgent questions. Check with your doctor or doctor’s Medical Office Coordinator (MOC) to find out his/her call-in times.

For urgent problems, please call our office any time.

Forms & Letters

You may bring forms or letter requests with you to an appointment or fax or mail them to us. Please indicate where your documents should be sent once completed. Remember to fill in any patient sections before you give us the form. Please allow at least 7 days for processing. There is a minimum fee of $10 for each form or letter that is not completed as part of your visit (higher for longer or more complicated forms).

Prescription Refills

If you need a prescription refilled, ask your pharmacy to call or fax the request to our office. This is the most efficient way to manage refills.

You may also call our main telephone number and leave a message on the prescription line.
Please provide:

  • your full name and date of birth
  • your phone number
  • the medication name and dose
  • how you would like the prescription processed.

If you want the prescription sent to your pharmacy, please provide their phone and fax number. If you prefer written prescriptions, they can be mailed to you or picked up at the front desk weekdays 8:00 am - 4:30 pm.

Please remember that controlled substances (e.g. narcotics and some medications for anxiety or ADD) require a written prescription and cannot be called or faxed to a pharmacy.

We require at least 2 business days to refill medicines. Please remember to arrange for refills before you run out.
