Once your appointment date is scheduled, an Executive Health Coordinator will fax or email a medical history questionnaire for you to complete and return as soon as possible. Your Executive Health internist will review the questionnaire to design a personalized program for you. You will then receive an appointment confirmation, complete itinerary, and pre-exam instructions about two weeks before your appointment.
For patients receiving care under a private contract, a registration fee is charged at the time the appointment is made. This will be applied against the final bill. The fee will be refunded if the appointment is cancelled at least 10 business days in advance; otherwise, the fee will be forfeited in full.
If you need assistance with travel or hotel information, your Executive Health Coordinator can assist you. If needed, language interpretation can also be arranged for international patients and guests.
