Requesting a Clinical Genetics Appointment

Call the clinic office at (410) 955-3071 to schedule an appointment. You will be asked to provide some preliminary information about the patient.

  • Patient's name
  • Patient's contact information
  • Whether this is a new visit or follow-up appointment
  • Reason the appointment is requested
  • Brief medical and family history
  • Patient's health insurance information
  • Contact information for the patient's primary care physician and referring physician

We will do our best to schedule your appointment as soon as possible with the most appropriate provider. On average, we have appointment availability within 2-3 months, but in some cases the wait may be longer. Of course, emergencies can be accommodated on the same day, if necessary.What else will I need for the appointment?

MEDICAL RECORDS: Copies of certain medical records that may assist us in the genetic evaluation may be requested. These records should be mailed to our office at the address below, be faxed to our office at (410) 614-9246, or brought to the appointment. Extensive records should be sent in advance of the appointment for review. Medical release forms are available to assist in record collection.

FAMILY HISTORY INFORMATION: Also note that because an extensive family history will be taken at the visit, it is helpful for the patient or the patient's parents to find out as much as possible about the pregnancy and medical histories of family members prior to the appointment.

INSURANCE REFERRAL:  We request that patients are active in obtaining appropriate referrals for the genetics evaluation and any accompanying lab or radiological studies. If insurance referrals or preauthorizations are not in place at the time of the appointment, the appointment may have to be rescheduled.
