Before your first appointment or consultation with Drs. Boahene, Byrne or Ishii, we recommend that you print out and complete our New Patient Form. This will help expedite the check-in process.

 Additionally, please review the patient information documents about procedures we provide. This will give you a better understanding of what is expected of you before and after the treatment or surgery has taken place.
