Freedom from Smoking Program
The Freedom from Smoking Program is a group-based smoking cessation program designed to help smokers plan a successful quit attempt by providing essential information, skills for coping with cravings, and group support. The eight classes incorporate the most current behavior modification models/theories for the most successful smoking and tobacco cessation outcome.
Diabetes Conversation Map Training
This five week course is designed to engage small groups of people (3 to 10) with diabetes in an open discussion about their disease. The training will give basic diabetes education and will enable participants to learn from the knowledge and experiences of one another. Each session is designed to be fun, interactive, and full of learning.
Chronic Disease Self-Management Program
This workshop is facilitated in two and a half hours, once a week, for six weeks. People with different chronic health problems attend the program. Class topics include:
- Techniques to deal with problems such as frustration, fatigue, pain and isolation
- Appropriate exercise for maintaining and improving strength, flexibility, and endurance
- Appropriate use of medications
- Communicating effectively with family, friends, and health professionals
- Nutrition
- How to evaluate new treatments
Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. The participants learn how to implement a weekly action plan to take small steps toward reaching their ultimate health goals.
