Financial responsibility must be established prior to test initiation. Testing will not be initiated until billing issues are resolved.

Billing options include the following:

	Institutional Billing (If paying by institutional credit card, complete the Credit Card Authorization Form).
	Patient Self-Pay (click for information and forms)
	Insurance Billing - Contract Managed Care (click for information and forms)
	Maryland Medicaid (click for information and forms)
	Medicare (click for information and forms)

We are not able to bill insurance plans with which we are not contracted to provide laboratory services, even if the plan authorizes testing.
