Patient Information

The Johns Hopkins Comprehensive Diabetes Center is pleased to take part in your medical care. Listed below are some phone numbers and information.

TEST RESULTS:

Please try to go to the lab 7-10 days before your appointment for testing if a labslip was given to you at your last visit.

Blood tests and other study results obtained after your visit should be completed, interpreted, and ready for review in two weeks. We will then write a letter reporting them to you, including a copy of your results. If any results need prompter action, we will contact you immediately, by phone.  If you have not heard from us in two weeks call our office at 410-955-7139.

PRESCRIPTION REFILLS:

Please have your pharmacy fax a refill request form to 410-614-9586.  You may also call our office at 410-955-7139 to have your prescription electronically sent to your pharmacy.  Please allow 48 hours for the pharmacy to complete your prescription. 

BILLING QUESTIONS:       

            For billing questions about your Hopkins doctor’s bills, please call 410-933-1200.

APPOINTMENT INFORMATION:

            Diabetes is best managed by a team of health care providers. We believe strongly in the value of non-physician professionals, and we may ask you to see Nancyellen Brennan, Erica Hall or Susan Renda, Nurse Practioners and /or Emily Loghmani, R.D. All are certified Diabetes educators. These talented professionals have much to offer to help you manage your diabetes. For new patients, we have put our New Patient Questionnaire online if you would like to print and fill out the forms before coming to your appointment. 

Please be sure to bring your glucose meter AND an updated  list of medications to your appointment. We use glucose readings to make adjustments in your treatment plan.

If you have an emergency please proceed quickly to an emergency room OR contact the Endocrinologist on call at 410-955-6070.
