After
notification of acceptance, submit the following
forms (as appropriate) along with the blood
sample.
A submitted sample
will not be analyzed until all completed forms
have been received and both the clinician and
patient have agreed to follow-up.

the Subject
Consent Form
(to be signed by the subject or
parent/legal guardian if subject is a
minor)
- Download the Pediatric
Assent Form
(to be completed by subjects age 8 to 18,
must be accompanied by the subject
consent form signed by parent or legal
guardian)
- Download the Family
Member Consent Form (to
be completed by/for parents, siblings or
other relatives
if they are invited to participate in the
study)
