1) Where do we send
completed patient consent forms and laboratory
request forms?
Completed
forms should be sent to Barbara Karczeski at the

2) What color/type of
tubes should I use for drawing blood?
We prefer
between 5 and 10 cc of blood to be drawn in EDTA
(lavender top) tubes.
3) Are there any
specific shipping requirements?
Samples
should be shipped at room temperature by Federal
Express or other overnight courrier. Blood
samples are stable at 4 degrees Celcius for a
week or longer. Please call +1 (410) 614-6911

appreciate being notified by phone when a sample
is being shipped.
