Continuing Care Program
The Continuing Care Program offers the finest ongoing medical care,
education, and support services for persons with HD and their families.
The continuing care clinic currently meets on Wednesday mornings
and provides ongoing HD treatment and symptom management through
environmental interventions, medications, education and counseling.
Patients are seen by a neuropsychiatrist specializing in HD and
also seen by our social worker, Kit McFarlane. A variety of additional
services are available including occupational therapy and speech
and swallowing evaluations.
Our social worker, Kit McFarlane, is available for consultation
both during clinic hours and outside of clinic hours, 3 days a week,
currently Mondays, Tuesdays and Wednesdays. She offers evaluation
and counseling and can assist in disability applications and nursing
home placement. In addition, she runs a support group on a monthly
basis.
Our multidisciplinary team works with patients and families to
develop a personalized program of medical care. Community referral
and case management services are integral components of the treatment
team's approach. Patients are required to have a primary care physician
in the community for non-HD related medical care.
The services of this clinic are provided on a "fee for service"
basis, private and government insurance is accepted. Because the
clinic is supported by the Huntington's Disease Society of America,
we can apply a sliding scale based on income and provide services
for indigent patients and families.
