Make an Appointment

Physician referral is requested, although not mandatory, for patients with commercial insurance, but a referral background letter is preferred. For Managed Care/HMO patients, your Primary Care Physician must refer all patients to the clinic. Please fax or mail referrals to the Referral Coordinator. Referrals should include permission to perform skin testing and pulmonary function testing. As soon as the referral is received, the Referral Coordinator will contact the patient to arrange an appointment. Please be sure to bring any pertinent medical records and x-rays to your first appointment.
