How to Refer Patients

To initiate the preadmission process or to request an inpatient physician consultation, simply call the Johns Hopkins Department of Physical Medicine and Rehabilitation at 410-532-4701

To have a non-hospitalized patient evaluated for admission, please schedule a clinic appointment by calling 410-614-4030 or 410-532-4714

Steps in the referral and admission process:

  1. Referral to rehab
  2. Physician or R.N. screening evaluation
  3. Information sent to admitting/accounting
  4. Insurance verification and precertification
  5. Information sent to PM&R point person
  6. Unit is contacted, time and bed are assigned
  7. Patient is admitted

Note: Because the Comprehensive Inpatient Rehabilitation Unit is a separate program, Johns Hopkins inpatients who come to our unit are technically discharged from the hospital, then newly admitted to the rehabilitation unit. A physician discharge summary and discharge notes from other acute care team members are required for admission to the Rehabilitation Unit. An interagency referral form is not necessary. The medical record should always accompany the patient when transported to the Rehabilitation Unit, where it will be processed as an old chart and a new record initiated.
