- If you have a question about accessing a patient record, you will need to report the name of the patient are you accessing/trying to access and your relationship with them.
- If you have a question about a referral, you will need the name of the patient you referred and, if possible, Referral ID.
- If you have a question about a security issue, please have available the names of other users like you at your site.
